This is a printout for the client. Before contacting an investigator, we highly recommend that you print this and keep it handy during the entire investigation starting with the initial contact. We encourage you to rate each part as accurately as possible and to write some notes for feed back. This will only be visible to the investigator after there are 3 ratings for that group. They will only show collectively so that the ratings are anonymous to the investigators. This way you can be honest as can be.

Instructions: Once this is completed, you will need to log in and click on the 'Fill out report card' on the main menu. You will be prompted for a card ID number, found on the investigators bio page. So be sure to go to the investigators bio page first and write the ID number down prior to doing the actual rating of this investigator. This number is for tracking purposes and to help us make sure these are not falsified.
